How to Make a Records Request

Please note that, in accordance with Texas law, Joyful Restoration Wellness has 15 calendar days from receipt of a proper written request to respond.

- Upon receipt of a written request from a patient to examine or copy all or part of the patient’s mental health records, the professional shall, as promptly as required under the circumstances but no later than the 15th calendar day after receiving the request:
- Make the information available for examination during regular business hours and provide a copy if requested; or
- Inform the patient if the requested information does not exist or cannot be located.
- Unless otherwise prohibited by law, a reasonable fee may be charged for retrieving or copying records. Records may be withheld until the fee is paid unless a medical emergency exists.
- No fee may be charged where prohibited under applicable Texas law.
